Pepper Gray bfc51e577f remove dead code (ObjectCacheRefCounter) ...

building using libc++ failes due to shared_mutex ObjectCacheRefCounter
inflating InternalThreadState beyond FEX_PAGE_SIZE, thus triggering
the static assert:
