Ryan Houdek 12dc16780f OpcodeDispatcher: Fixes FEX's H0F3A table handling of REX.W
Most of this table ignores REX.W, but two encodings change behaviour
based on REX.W. These two encodings are PEXTRD/PEXTRQ and PINSRD/PINSRQ.

For every other instruction encoding, they will ignore REX.W, but FEX
was requiring that they didn't have REX.W encoding. I had special cased
this in the past by adding PALIGNR, but that didn't handle any of the
other instructions.

We can't just handle REX.W in the OpcodeDispatcher and remove the two
special cased instructions because these vector operations also interact
with instruction prefix 0x66 which changes the operating size to 16bit
with regular instructions.

So instead just generate all listings of instructions with REX.W being
zero and one and install handlers in all cases.

Billy Laws 981c3009ee FEXCore: Emulate EFLAGS.TF
When set - either via POPF or a thread context operation - the trap flag
raises a single step exception after the execution of each instruction.
As e.g. a JUMP instruction with TF set will raise an exception at the
jump target. Handle this on the FEX side by storing both the flag itself
(in bit 0) and a 'block exceptions' flag (in bit 1, inverted). Each
generated block when TF is set is then forced to a single instruction
with logic to raise the exception at the start. Initially after setting
TF exceptions are blocked, then at the start of the block they are
unblocked so that after the instruction executes an exception is raised
at the start of the next block.

These weren't even wired up to the frontend. We aren't going to support

Ryan Houdek 37a70e2ec6 FEXCore: Convert Base tables over to constexpr
Only doing the single table for review purposes. Once reviewed I will
hammer out the remaining tables.

Similar to #3320, most of the OpcodeDispatcher tables can be consteval
and made to be a compile time constant. This just requires shuffling the
code slightly. The idea is to get almost all of the table setup out of
the `InstallOpcodeHandlers` function and instead only install the
handlers that change based on 32-bit or 64-bit, just like the x86 tables
we also did.

Alyssa Rosenzweig edf1a7970d X86Tables: add Literal() helper
Any time we get the value of Literal, we want to assert that it's actually a
literal. We've been open coding this pattern sporadically throughout the
opcodedispatcher. Let's add an ergonomic helper to fetch the value of literal,
asserting that the value is indeed literal.

Ryan Houdek bb308899b9 Frontend: Expose AVX W flag
Previously we could always tell the size of the operation depending on
how this effects the operating size of the instruction. Converting
64-bit down to 32-bit as an example.

AVX gather instructions are the first instruction class that can't infer
this information. The element load size is determined by the W flag but
the operating size of 128-bit or 256-bit is determined by other means.

Expose this flag so we can determine this difference. The FMA
instructions are going to need this flag as well.

Ryan Houdek 010028e381 FEXCore: Fixes the difference between CPL-0 and undefined instructions
undefined instructions are expected to return SIGILL, while implemented
instructions that aren't available in CPL-3 are expected to SIGSEGV.

Noticed this while testing out CPU-Z, it installs a kernel module and
does a bunch of `RDMSR` and `OUTS` instructions. Decided to walk through
the rest of the instructions in the `System Instruction Reference`
section.

Turns out there's a bunch of oddities in there that we don't support.
First step is to go through all the explicitl SIGILL and SIGSEGV and
implement a test for them.

Next step will be implementing the remaining operations that are
considered "System" operations but are still available in CPL-3.
